Report checklist

Formal points you can check before signing a report to the Gutachter (the expert reviewer). No email address, no sign-up: as a page to tick off or as a PDF to print.

Formal points only. The points correspond to the formal level-1 rules of the Gutachter-Check in the Application Copilot (planned for launch in January 2027); the identifiers (e.g. L1.konsil_missing) name the rule concerned. The checklist assesses neither the content of your report nor your treatment and says nothing about the chances of approval. The Psychotherapie-Richtlinie, the Psychotherapie-Vereinbarung and the current forms are binding. As of 4 October 2026. The PDF is in German.

Completeness of the report

  • Every section that only you can complete (e.g. diagnosis, treatment plan) has been completed. L1.field_missing
  • No section of the report is empty. L1.section_empty

Sources and placeholders

  • Every sentence is backed by a source in your case file or has been adopted as your own text. Sentences without a source have been sourced, adopted or deleted. L1.unsupported
  • No sentence says more than, or something other than, the source it cites. L1.unsupported
  • No pseudonymisation placeholder (e.g. “[DATUM]”) is left in the text. L1.unsupported

Check before signing

  • You have read every section yourself and marked it as reviewed. L1.sections_unreviewed

Requested hours

  • The requested phase (KZT 1, KZT 2 or LZT) and number of hours are stated. L1.request_missing

  • Hours approved so far plus hours requested do not exceed the approval step of the approach (VT 60, TP 60, AP 160, ST 36 h). If they do: the justification under § 30 sentence 2 PT-RL is in the report. L1.hours_over_regular
  • Hours approved so far plus hours requested are below the maximum of the approach (VT 80, TP 100, AP 300, ST 48 h). L1.hours_over_max

Short-term therapy and conversion

  • KZT 2 is requested after 7 sessions of KZT 1 at the earliest (§ 11 para. 5 sentence 3 Psychotherapie-Vereinbarung). L1.kzt2_too_early
  • Conversion to long-term therapy is requested by the 8th session of KZT 2 at the latest (§ 11 para. 5 sentence 4). L1.umwandlung_late

Konsiliarbericht (initial and conversion request)

  • The date of the Konsiliarbericht is entered and is not in the future. L1.konsil_undated, L1.konsil_future
  • The Konsiliarbericht was obtained before the start of guideline therapy (§ 32 PT-RL). L1.konsil_old

Diagnoses

  • Every ICD-10-GM code has a valid format (a letter, two digits, optionally a dot and further digits, e.g. F32.1). L1.icd_format

  • At least one diagnosis carries the suffix G (confirmed). L1.icd_no_g

Course of treatment (conversion and continuation)

  • The previous approval (for a conversion: that of the short-term therapy) is recorded in the case file. L1.prev_outcome_missing
  • The number of sessions delivered does not exceed the approved hours of the current phase. L1.hours_used_over_approved
  • Sessions without an assigned approval (e.g. Probatorik, the trial sessions) are known to you and intended. L1.sessions_without_approval

Questionnaires cited

  • Every questionnaire named in the text (e.g. PHQ-9, GAD-7) has a recorded value that can serve as a source. L1.measure_missing
