Insurer extension report checklist
Twenty points to check before you ask a client's health insurer for further sessions. No email address, no sign-up: read it on screen or print it and tick by hand.

Form only, not content. The checklist covers what a request usually needs. It does not judge your clinical reasoning and says nothing about whether the insurer will fund more sessions. Your insurer's current instructions come first.
Before you start
- Insurer's current instructions checked: portal, form, email or phone.
- Membership number and authorisation number to hand.
- Sessions authorised, sessions used and sessions remaining counted from your own record.
- Request timed so that a reply can arrive before the last authorised session.
- Client's explicit consent for this report recorded: date, insurer, what will be shared.
Content
- Presenting difficulties described as recorded at assessment.
- Your assessment of the current condition, in your own words.
- Treatment approach named; each goal linked to a presenting difficulty.
- Progress described with concrete, dated examples from session notes.
- What has not changed yet is stated, not left out.
- Outcome measures given as instrument, raw score and date, at the start and now.
- Number of sessions requested, frequency, and the reason for that number.
Accuracy
- Every statement about progress can be traced to a note or a score.
- Dates and session counts match your own record and the authorisation.
- Client name or reference and policy details match the authorisation.
- Nothing included that the insurer does not need for the funding decision.
Before you send
- Read through by you in full, signed and dated.
- Copy kept in your own client record.
- Date of the request noted, with a reminder to follow up.
- Client told what was shared and when.

Frequently asked questions
Do I need to send the checklist to the insurer?
No. It is for you. Use it to check your request before you send it or before you call.
Does this work for phone requests?
Yes. Bupa's published terms describe a contact with their team rather than a form. Prepare the same points in writing and use them as speaking notes.
Is anything stored when I use this page?
No. The page has no form. Nothing is sent, stored or tracked, and the page sets no cookies.
